Usually the form wants supporting documents from an MD.

Be careful how much time you use under FMLA, save some days in case you need time off for your own medical reasons.

Thank goodness I never used any of my FMLA when my Dad had surgery and was in rehab, and me driving Mom every day to visit him during what would have been my working hours. I used all my vacation days, all my sick days, and days without pay. A few months later I was diagnosed with a very serious medical condition that required major surgery and all of my FMLA days.

I had mine completed by my Mom's nursing home MD w/o charge.

I was going to say ask nicely and bring chocolates, but you should not have to. I do know some offices that charge for filling out forms, but most don't. And if I never had to fill out more multipage forms again I'd be thrilled, but honestly, as was nicely pointed out to me the last time I was heard complaining about them, how often do you have a chance to save a person's job just by writing a few phrases on a couple pages of paper?
