Certainly. Talk with the social worker and Admissions in the facility where you'd like to make the transfer to see if accomodation is available/possible. Talk to the social worker in the current facility.

If your loved one is adjusted to a facility and relatively comfortable there, and you are happy with the care? I would move heaven and earth to keep from a move.

Just quick caveats. If the move is only to be closer to family, she or you/family may be asked to pay for transport. If she is on Medicaid, it does not cross state lines. She Might have to requalify. A move may trigger an abrupt decline. Last, will family actually visit????

I have read your other posts. I get the impression Mom was in the Hospital and needed further care. Was she placed in a NH? And if so, why was she not placed in one closer to family?

Who do you need to request? Mom is not in jail? If there is a POA or a guardian that person finds a facility closer to family and sets up a transfer. If on Medicaid, going out of state would be the problem. But by writing as you did it gives me the impression the State maybe her guardian.

In reading your other I noticed that you did not answer thecquestions asked. It really helps us to give u better if you can come back and asnswer the qiestions otherwise we assume.

I really feel you need help in navigating all this. There are Social Workers who can help you with understanding the whole process.
