I assume you are talking about Medicare and not Medicaid, which are 2 different programs entirely.

What happens if I miss my Medicare initial enrollment period?

If you missed your initial enrollment period when you first became eligible for Medicare, you can only sign up for Part A and/or Part B during the General Enrollment Period, which runs from January 1 to March 31 each year.

Penalty will depend. There is something about when you are working and the employer is paying for your health insurance. I think you need to call Medicare.
